Once you have received your training, you will receive two forms to complete. The first one is an EMHC Agreement Form. On this form, please select at which Mass(es) you would like to assist. The other form is a Form of Good Standing. Please complete the forms and email them back to the office. Once we have receive both forms, we will submit your name to the Diocesan Chancery for Commissioning.
